Finding a Doctor: Challenge for Patients with Anemia

Anemia is not a diagnosis – it is a symptom. Finding the reason for anemia can occur in a matter of moments, months, even years.  The process of investigating, ruling out causes and testing methods, altering diets, supplementation, undergoing iron injections, infusions, or whole blood transfusions might fail to eliminate the anemia.  

 Doctors of all disciplines are capable of diagnosing and treating anemia. However, during the journey to locate and cure the cause, a patient with anemia will likely see one of two types of specialists: a gastroenterologist or hematologist.

 The gastroenterologist will look for causes associated with (but not limited to) blood loss from the digestive system or problems of absorption. Some of the causes they will pursue can include celiac disease, H. pylori infection, bowel lesions or polps, hemorrhoids, and esophageal variances (varicose veins of the esophagus). They may include evaluation of the kidney function for the production of erythropoietin (EPO) – a hormone produced by the kidney that stimulates the bone marrow to make blood cells. These investigations can involve scopes, blood work, urine samples, biopsies, and scans.

 The hematologist will look for causes associated with the blood forming organs and the systems that manage misshapen blood cells or ones that are no longer of use. Blood work can provide clues to some of these causes, but generally a bone marrow aspiration procedure will be required.

Pulmonologist put me through over $4000 of tests (paid by me because I have a high deductible plan) with no negative results. PCP diagnosed severe anemia after routine blood work. Pulmonologist knew I had anemia and did nothing about it. My PCP had me in with a hematologist immediately, and hematologist had me in the hospital the next am for two pints red blood cells, followed by two units of iron. Both said I was at risk for a heart attack. Tests to determine cause of blood loss showed ulcers and gastritis. Now take Protonix for that. I feel like a new person. Your brother should check with a hematologist if he is having these problems.
